From context, it looks like the user plans to abuse the drug, and the pharmacist is well aware the user plans to abuse it.

I know its memes and not much depth is expected, but I think its slightly more.
As in, Pharmacist is a socially acceptable profession. Xanax is a socially acceptable pharmaceutical. It is socially acceptable for a patient to take this medication. However, this particular person is planning to abuse the drug, while still maintaining social acceptability, and the pharmacist knows this too, and is giving a wink and a nod they are aware of it.
The joke being "we're not dirty drug dealers and users, but really, for this case, we are, and we're getting away with it being socially acceptable."

I'm prescribed Xanax, but the lowest dose. It's for my panic attacks, and it is an extremely fast acting, very effective drug that calms you down. I kind of just makes you not care about much and a little drowsy. I can imagine that in large doses, it would make one feel pretty damn good.
